Some Info about PASSPORT
PASSPORT is a Medicaid Waiver Program, Home and Community Based Service Waiver, based on the philosophy that no older person should be forced to accept nursing home placement because of the lack of available home care services. The program provides screening and assessment of persons, age 60 and over, at risk of nursing home placement. Enrollment in PASSPORT is based on the individual meeting eligibility criteria, including: financial, health, and safety among others.
A case manager, provided by PASSPORT, develops a Plan of Care with the client, caregiver, and community agencies to provide and manage a mix of services. The Plan of Care, which must be approved by the client’s physician, includes all of the services the client receives. The case manager monitors and adjusts the services delivered to assure the client’s continuing health and safety needs are met.
